February 18, 2022
Drs. Robert Karman, Courtney Minor, and Warren Shaikun will no longer be practicing with Expert Sleep Medicine, nor affiliated with Family Allergy & Asthma, effective April 1, 2022. These three doctors, along with Tiffany Helmerich, APRN, will be joining University of Louisville Healthcare as a part of their Sleep Care department. As a result, Expert Sleep Medicine will no longer be operational. We apologize for any inconvenience this may cause.
Patients who have been seen in the past 6 months or have an appointment scheduled in the next 6 months will have their medical records follow their provider to UofL Health for continuity of care. There is nothing you need to do for this transfer to occur. UofL Health will be reaching out to patients with appointments currently scheduled after April 1, 2022 to reschedule those.
If you have not be seen since October 1, 2021 and do not have an appointment scheduled but would like to continue to see one of these providers, please contact UofL Health directly.
If you would like to move your medical records to another provider besides UofL Health, please visit familyallergy.com to download and submit the medical release form, found in the Patient Center section of the website.
If you are utilizing our durable medical equipment (such as CPAP or face masks), we will be in touch prior to April 1 to transition these services to another provider. You do not need to do anything at this time.
